225ac60cf4 Add map/4, foreach/4 with query parameters (#67) 
- Function signatures wrt the non-parameterized forms are analogous to `q/2`,
  `q/3`.

785903ae6d Keep a reference to the connection before the prepare statement is put on the command queue. 
This should fix a race condition when a prepare is called right before the
db connection is garbage collected.

c7c9421429 Pass back errors in fetch steps to the caller 
Constraint errors (e.g. foreign keys) can fail in the fetch phase of a
query. The library did not support error handling in the fetch phase yet.
