Keep a reference to the connection before the prepare statement is put on the command queue.
...
This should fix a race condition when a prepare is called right before the
db connection is garbage collected.

...
sqlite3_column_count() may return 0 for SQL statements that do not return rows
from the database. This should not be treated as an error.

Pass back errors in fetch steps to the caller

Constraint errors (e.g. foreign keys) can fail in the fetch phase of a
query. The library did not support error handling in the fetch phase yet.
